Preventative Care

Our providers are optimally suited to provide primary care for the adult patient. They have trained hard to become experts in disease prevention, early detection, and health promotion. Their focused approach can help you. Research shows us that many opportunities to prevent disease and to prevent complications of existing diseases are still missed – we want to remedy that. From cancer screening to preventing cardiovascular disease to PrEP, we want to keep you healthy!

Cardiovascular Health

CDC data lists heart disease as the leading cause of death in the US as of 2015. We are passionate about preventing heart disease, or if you have it, treating it aggressively. We stay current on the latest evidence-based care, including exercise, nutrition, medication, and advanced lab testing and imaging.

Reproductive Health

Wise Patient Internal Medicine provides contraception counseling, routine pap smears, breast exams, and diagnostic gynecological exams for symptoms you may be experiencing. We follow the US Preventive Task Force guidelines and the American Society for Colposcopy and Cervical Pathology guidelines for making sure you are up to date on cancer screening for your age group and medical history. If your have symptoms that require advanced care or further imaging we work closely with trusted gynecologists to ensure your needs are met.

We provide services for Long Acting Reversible Contraception (LARC) including IUD insertion and removal, Nexplanon insertion and removal and injectable birth control.

Mental and Behavioral Health

The U.S. Department of health estimates that only about 17% of U.S. adults are considered to be in a state of optimal mental health. Your care at Wise Patient will always be composed of both physical and mental health.

We work with you to figure out what resources will help your specific situation, whether that means medication to treat your condition, or a referral to a therapist.

If you have previously been diagnosed with ADD or ADHD our providers will help manage your medications after reviewing your past records confirming your diagnosis. Please have your records with you at your first appointment or faxed to us before your appointment. We do not start patients on stimulants without thorough evaluation from an outside licensed mental health provider.

Weight Loss Medicine

You will never hear us say, “Just eat less, and exercise more!” Weight loss is complicated, hard, and greatly determined by your genetic predisposition. Everybody is different. The rapidly growing field of obesity medicine research has just begun to illuminate the complexities at hand. Our providers are board-certified in Obesity Medicine and take an evidence-based approach to nutritional counseling and medication management.

Medically supervised weight loss can ensure you are losing weight in a healthy fashion. Using our body composition scale at each visit, we can ensure your percent fat is decreasing and not your muscle mass.

Gender Affirming Care

We provide affirming services for transgender, genderqueer, non-binary, and all gender expressions via the Informed Consent Model. No letter from a therapist is needed.

We provide hormone therapy (HRT), pre-operative medical evaluations, required letters for gender-affirming surgeries, letters for Social Security and Passport name and gender changes, PrEP, and of course, whole-person primary care.

We have a gender-neutral single-toilet bathroom, and a small quiet waiting room.

We understand that gender is a spectrum and that there is no one-size-fits-all hormone regimen, and we will work with you for hormone therapy that fits YOU.

HIV Prevention and Treatment

Nazlee Navabi, MD and Janna Cuneo, ARNP are accredited by the American Academy of HIV specialists to manage primary care and HIV across the lifespan. All our providers actively manage PrEP and HIV prevention services, including sexually transmitted infection testing and treatment, and injectable and oral medications.

Diabetes

Does diabetes run in your family? Have you been told you are “pre-diabetic”? Are you worried about insulin resistance? Or do you already carry a diagnosis of diabetes? We will work closely with you to manage your glucose and prevent complications, through exercise, nutrition, and medication (including insulin management if required). We manage the care of most people with diabetes in-house and find that our availability combined with our health coach horsepower means it is rare to need to refer outside to an endocrinologist.

They are specially trained to solve puzzling diagnostic problems and can handle advanced chronic illnesses and situations where several different illnesses are interacting with one another. When other medical specialists are involved, our providers seamlessly coordinate their patient’s care and manage difficult medical problems associated with that care.
